Couple more tie rod end questions

Is there supposed to be a washer under the tie rod end nut? It's been several years since I took the tie rod off the truck, so going by memory is no good, and there were no washers under the nuts where I was storing the tie rod (with rod ends attached), impying that there are not supposed to be any washers. However, when I install the tie rods onto the steering knuckles and run the nuts up on them (onto the ball studs), the cotter pin holes in the ball studs are well above the tops of the castellated nuts, not lined up with the slots in the nuts. This makes me think there is supposed to be a rather thick washer there, but my parts manual doesn't show anything other than the nut.

Finally, my tie rods have the seals that use the flat metal washers and flat rubber washers and the springs. Is there supposed to be a flat metal washer on either side of the rubber washer (one above and one below), or just one metal washer on top of the rubber one? My parts manual has two different views that seem to show it both ways. The left rod end had metal washers above and below the rubber one. The right side didn't have a seal of any kind, so I bought a kit that had seals for both rod ends, but it only had two metal washers in total, which seems to say that there is only supposed to be one metal washer for each rubber washer.

Any help with these questions would be appreciated.
