Steering Problem, Help!

I installed my steering box and it's not lining up correctly. As you can see in the pic (and slideshow), it appears the steering box is mounted correctly, no foreign objects, flush with the frame.

Also, you can see that the output shaft comes through the frame "square". The steering shaft is offset slightly where it passes through the firewall, but much more at the dash?

The throttle rod is exactly centered in its hole and the cab appears to be centered on the frame? The firewall hump is centered over the bell housing nicely.

It all fit the way it was supposed to before I took it apart. Any suggestions? I'm stumped.
