I think it's been 17 years since the last one. We are in a frontier with shuttle flights. I'm sure DTS/FAA and others will gather data to piece together the puzzle that resulted in it's failure. With the number of years and flights that have taken place, NASA really has done an excellent job ensuring the safest flights possible. My guess is a part had reached its service limit and failed.
We have to view those individuals that have been previlage with the opportunity to have pioneered these flights as our heros. They have made the ultimate sacrifice, while paving the way for future travelers. So many new products have come out of the Space program that have benifited countless people around the world. It's always with great sadness hearing of an incident of this kind. Lets keep them and their families in our prayers in the days to come.
