The non-sycro tranny will whine 1st through 3rd. If you have a 4th gear whine, then you have several possibilities:

1. Low oil in one or more drivetrain units (produces more gear whine),
2. No rear motor mount rubber insulators (engine lays on frame mounts and you will hear every vibration/whine in the drivetrain (metal on metal)),
3. Fatigued rubber cab insulators (Allows cab transmission floor cover to lay on top of transmission producing the same effect as 2,
4. If you have a PTO, the Idler gear and shaft are excessively worn producing bad gear vibration.
5. Worn units in general.

Look for any metal to metal contact between drivetrain and cab such as PTO lever vibrating against floor cover.

In 4th, I hear no whine or gear noise, only some rattle in my shift lever (worn) and gear rattle from the PTO from a worn idler gear and shaft (a couple future projects). But mostly, I hear just the whine of the tires, and the purrrrrrrrrrr of the engine.
