I am not sure where the peddles came from or who make them. They were in the truck when I bought it. I have seen them in other trucks from time to time. They fit an older style master cylinder that had a single reservoir for the brakes and clutch. The peddle assembly is not the critical issue, it's how am I going to clear the engine with the master cylinders now that they are mounted on the firewall. This is not too much of a problem with an inline 6, but with the V8 the valve cover is very close. I ended up buying a 5" diameter boster and brake master cylinder assembly. This was purchased through a shop the specialized in customized trucks (they do have a catalogue. The master cylinder is a cast iron one which apparently was used in a Corvette. It can be used with either disk or drum brakes due to its large reservoir size. When my installation was done I had about 1" between the valve cover and the vacuum boster. I have the original brakes on the truck and it stops quite well. I will be putting front disks on soon.

A second issue was how to get the center lines of the brake master cylinder and clutch master cylinder only 3 1/2 inches apart. Obviously with any vacuum boster I could not mount them sid to side. I resolved this by mounting a plate to the fire wall which had a 5" long heavey wall tube welded to it and a second plate mounted to the end of the tube. I mounted the vacuum boster to the second plate which put the booster about 5 1/2" out from the firewall. The clutch master was mounted on the plate that bolts to the firewall and is very close to the tube supporting the vacumm boster and brake master cylinder. I had to lengthen the push rod on the brake vacuum booster. It was done with threaded rod and rod couplers and set nuts and tack welds. The rear of the vacuum boster fits into the support tube.
