OK lets assume that the adjustments are correct. What truck is this on. If it is a flat fender does it have the synchro 3 and 4th gears (NP 420) or is it the older transmission that is totally non synchro?

Other things to consider. Did you get a defective clutch disk (ie is it warped)? If your clutch disk is warped, it will always be hitting someplace on the fly wheel. Did you have the fly wheel surfaced? It may be warped, more unlikely but it could be. Does this truck (you have not indicated what it is yet) have a hydraulic clutch. If so, you could have air in the system which could be preventing the clutch from fully disengaging. Bleed the system again even if you have done it a couple of times. If it is a hydraulic system, either your master or slave cylinder could be worn and allowing fluid to seep past the piston. One way to check is is to start the engine and depress the clutch and hold it down. Then shift gears and not how much griding your getting. Keep the clutch depressed for several minutes and try shifting again. If it becomes more difficult to shift as time goes by then I would suspect a worn master or slave cylinder.
