My 78 W150 came with a 400 big block and a 445 transmission. It is syncro in all forward gears, but Does Not have the "granny low" (unsyncro) first gear like in the 435's. I think they were found more often in "recreational" type trucks that were going to see a lot of highway use. Mine sees a lot of time in low range at the farm, because it doesn't go slow enough in high range for yard chores. I have had a lot of Dodge trucks, all of them with 4 speeds, and this is the only one that has the 445 instead of the 435, so they are definately a rarity. The 445 also has 2 PTO ports, one on each side of the case, while any 435 Ive seen has only the one on the left side.
