Do you have a fuel filter installed? How clean is the fuel tank? What is the condition of any rubber lines in the system?
I have seen rubber lines suck shut or swell internally and filters begin to get clogged not allowing adequate flow to supply under load. Any junk in the tank can collect on the fuel pickup and restrict the flow until the engine is stopped and the debris falls off.
Chris mentioned the fuel vent already, maybe take the truck for a ride without the cap on.
