I imagine there were a few different manufacturers. I bought one off eBay a couple of years ago, which was made by Nicson. I'd never heard of them, but I think the manifold was probably made many decades ago. I just got lucky in my search on eBay at the time. Since that time, I've done similar searches once in a while (just out of curiosity) and haven't come up with much. The dual intakes for the long engines (251, 265, etc) are not as common as for the shorter engines (218, 230, etc.).

You can also try George Asche. He is an old-time flathead guy, who has partnered with another guy, Tim Kingsbury, to make some tri-carb manifolds under the name AoK Boys. George can also make convert your original single-carb manifold into a dual carb manifold for what I recall being a very reasonable price. George's number is 814-354-2621, and he is located in PA. He doesn't do e-mail or internet, so phone is the only option.
