The square one goes under the bell-housing and above the frame bracket. The is a metal spacer that goes on top of the insulator and under the bell-housing, 9-71-12.
Used on trucks, Left/Right brackets (after serial No. 83919230 Det. Built 88766099, Cal. Built.). I don't know if they can be used on the earlier brackets. If the earlier brackets have a 7/8 holes, they should work.
