Is the cab transmission floor cover riding on top of the transmission? Look around the shifter opening. This is caused when you have fatigued rubber insulators or not enough insulators under the cab. This will transfer the running gear noise into the cab, very noisy, this may be your problem. you want at least 3/8th gap. If you have enough clearance, you may have a bad ring and pinion in the rear end.
