I would suspect the input or output bearing they are taking the load in 4th. It could be the counter shaft rattling as you suggested. My before trans which is basically the same had groves in the counter shaft from the roller cage bearings so it was really rattling. Obviously the inside of the cluster gear was also worn badly.
