The A frame usually attaches with dog ear brackets on either the top or front of the bumper, and hangs at the top from a cable that goes back to a headache rack. The telephone and power co trucks were modified when new by coach builders or fab shops. They usually have sections of the truck frame reinforced and a slip-jointed rod thingamajig to keep the A frame from falling back into the truck. Lots of different setups done by farmers, loggers, etc. I have the A frame off my power co truck, but I'll dig up some photos I've copied.
