Sounds like a problem related to the input gear of the PTO. It is the only gear that turns when the PTO is in neutral. It could be a tooth that is out of spec, or a problem with the bearings.

You could try this: With the engine off, put the transmission in neutral, engage the PTO, depress the clutch (someone else will have to do this), and turn the PTO output shaft by hand, or with a large drill motor. You may be able to hear or feel when the tap occurs. Remove the PTO and see if there is anything obvious on the transmission idler gear, or the PTO input gear.
