Often find the holes are damaged from nuts loose or general wear and tear leading to cracking. Ideally when repairing you grind out and weld, may also need to run some extra weld right around the holes and get them machined back to specs - sometimes holes are so badly worn there's no chance the nuts will hold the rim properly.
