Really check out the PTO. I wish there was a way to actually hear the knock, it could tell us a lot. If there's a way to spin it up like dc mentioned, it might show the problem. A lathe or big drill motor? Lock the case down and power in through the input shaft, both directions and in forward and reverse.
If nothing shows up it's possible that the noise is between the PTO and the transmission. Take some time and get the lash dead-huts. 0.008-0.010 is your target. It's a pain, but it's worth it. Sometimes it's easier to remove the other PTO cover and send the feeler gauge that way. Also check the bearing in the counter-shaft. The PTO may be aggravation a bad bearing there.
