Loose plugs will do that sometimes, they make a light tapping sound and if you don't identify and tighten, it doesn't take long before the threads strip out. Sometimes you can rethread the head to get any old plug thread out, usually the threads on both are pretty much toast. Not saying that's the problem but I've seen it before.
