Here is how I did it. I take a chisel and mark an X where the center of the hole should be (holes are usually worn oblong). Make sure that the marks go well beyond the edges of the holes. Now weld up the holes being carefull not to destroy your marks. Do an initial grind of the weld making sure to not destroy your marks. Mark the center for the hole based on your marks and drill. Now finish grinding. I have done this on 4 PW doors and the 4 hinges on my carryall tail gate. It works great.
