Ray Suiter Adapter Plate for NP435 Transmission

For those of you who used Ray Suiter's 318/360 conversion kit with an NP435 transmission, do you know the measurements for the adapter plate that bolts to the side of the NP435? This adapter allows you to attach your original transfer case and E-brake levers. My adapter is gone and I'm hopeful I can fabricate something so I can attach the levers.
