By the way is this a 1/2 or 3/4 ton command car? I assume that it has the original non-synchro transmission and either the 1/2 ton chain driven transfer case or the 3/4 ton gear driven transfer case.

The next step will help determine if it is the transmission or Transfer case. Disconnect the the short drive shaft between the transmission and transfer case. Start the engine and put it in gear (try all 4 gears as before). Is the noise still there? Is it less or more? Block the wheels so that the truck can not roll and lay on your back looking up at the transmission when the truck is running and in gear. Try to pinpoint the noise. Do not stick your hands anywhere near the transmission!!!!

Reconnect the drive shaft between the transfer case and the transmission. Now disconnect the rear drive line at the transfer case. Make sure the truck is in 2WD. Start the truck and put it in gear. Listen to the noise from both the driver's seat and from underneath as before. Try to pin point the source of the noise. Note any changes or differences from the first time you were listening to the transmission alone. Keep your hands away from the transmission and transfer case. Be sure to listen to the drive shaft U Joints.

I have a noise in my Carryall and by doing the above I was able to determine that it is the fron bearing of the transfer case.
