I ran across this reply to a post I made a few years ago, with the same question. It's a very nice description, and would be even more useful with some pictures.

My 1968 W300 had a Braden Co. winch shift handle that acts like a lever to engage and disengage the PTO shifter. It is mounted on the rear of the transmission "hump" to the left (driver's side) of the transmission and irectly behind the gas pedal, kind of close to the bench seat. It's pivot point (just under the floor) is located closer to the bench seat than the pivot point for the treansfer case levers. Underneath the cab, there is a solid rod (of which the center section is angled away from the transmission, with the front and back ends parallel but offset by the angle from one another) going from the bottom of the shift lever to the 10 bolt PTO on the transfer case. This is not the standard "T" handle and cable set up like Aaron's but nevertheless appears original to my truck, which was ordered by the federal government (U.S. Bureau of Mines, Department of the Interior).
