These crew cabs are very rare. You have seen pictures of most known ones on web page that you referenced. The only rarer Power Wagon I believe is the woodies.
The best way to find one of these crew cabs is to have it custom made. The frame and all is the same all they did was shorten the bed and extend the cab. I believe that the rear cab mounts rest on the rear spring mounts.
