It was a new disk, but the material seems thin and probably added to the failure. The tempered steel cushion spring that goes between the friction material disks, was rather thin and had some stress cracks in it. The riveted stop pins may have slowly become loose causing it to eventually break a part. I think it's like a lot of other products, less materials to keep the cost down and not enough used to indicate a problem.
