Definitley sounds like bad threads on regulator or tank and will not allow for a sealed fit. Always keep your tanks out of the work area and away from sparks. Used equipment not repaired or inspected by a reliable gas supply company can be dangerous and flashback arrestors are a must.I have used hand torches and oxygen lances for over 35 years on major industrial heavy demolition projects.
