My guess is that the flap was caught under the tire beads when inflating the tube causing it to be pulled to the center by the tube resulting in jagged tears. When you assemble with a new flap, make sure the flap is between the tube and tire. Put enough air in the tube to form it, then go around the flap between it and the tire with your hand to ensure it lays flat against the tube - no folds. Leave the air in the tube and place the assembly on the rim.
