Make sure the indication is accurate. Raise the number 1 cylinder to TDC on the compression stroke and see if the pointer lines up with the TDC mark.
Who knows what work has been done on your truck? I think timing covers are interchangeable on Dodge trucks, but the pointers are in different locations.
