I can't see a head gasket or intake leak letting enough water to lock one cylinder in the one stroke that would be available. I could see a cylinder filling when shut off, then causing grief when other cylinders fire at re-start.
I recall a boat motor V8 big block that had twisted a rod about 45°. 30 years ago, but IIRC, one rod, piston, rings did the repair. Rods is lots softer than cranks.
