On 3 or 4 occasions, the floormat in my '06 Dodge 2500 CTD has somehow got jammed against the accelerator pedal, causing it to accelerate. It's usually happened in 6th gear, so it's trying to get up to pretty good speed. The first time it happened, it was pretty weird. Simply reaching down and pulling it back solved the problem.
