My money says it was caused by hydrogen embrittlement. Electroplating is a common source of hydrogen embrittlement, especially on the higher grade fasteners. The fasteners must be baked after plating to relieve the embrittlement. Typical specs call for baking at 375 - 400 degrees F for four hours. All it takes is a lazy plater who skips the baking process and you wind up with junk fasteners.
I worked in electroplating for 22 years and I know this is a too common problem. I remember customer service getting a call from a customer one time about the heads popping off of bolts on assembled equipment while it was sitting on the loading dock waiting for shipment. That took some fancy splanin' by Lucy if you know what I mean.
