You work the spare onto the bar that is still attached on one end, then lift the bar until the looong stud fits through the hole. Many trucks have a key-hole, the nut stays on the stud. Hook the nut, tigthen it up. Also many later trucks have a tube handle on the end of the bar, makes it lots easier to lift into position.

I've driven my daily driver pick ups approximately 1,000,000 miles and never lost a spare. Including up and down Baja, and from Gonzaga Bay to Percebu in a two wheel drive F250. Didn't shake it off there either.
