Yep, that's all it takes. The wheel offset is sufficient for dualies without needing any spacer. The only reason I can see to do it, though, would be for flotation on sand or mud with a heavy load. The standard tire/wheel is already as strong as the axle so you'd gain nothing in load carrying capacity on normal terrain.
I toyed with the idea when I got my first M37, and quickly realized there was no point in it. Given the propensity for breaking axle shafts anyway, I wouldn't want any extra inertia out there spinning when bouncing over rocks and such.
