It appears that they haven't been made for years. 3/8" is the most common size on the trucks, but there are many others. I agree MWM probably has some. Someone on this forum a few years ago intimated that he had bushels of them that he scored at some surplus store, but seemed rather sketch about selling them. I found one box of smaller ones on ebay for not too much money. The last I checked, McMaster-Carr still had some for sale, individually and at a very high price.

I did track down the company that made them, West Coast Lockwasher, about 5 years ago. They had numerous sizes, and often several thicknesses of each size, in their catalog, but I don't think they have any in stock, or if they did, they weren't the sizes we commonly need. At that time, they still had the dies for _some_ of the sizes and would do a special run if the quantity was sufficient to justify it. For the other sizes, they have the drawings and would make new dies, but would need a REALLY big order or a lot of NRE money. I've often thought about getting back in touch with them and finding out just what it would really take to get a good quantity of at least the 3/8 and 3/16 washers, ideally in the correct thickness. I suspect that once I'd paid up front for a run of a million or so, the guy with the garage full that he bought surplus in 1965 would flood the market and I'd be out all that money.
