See mu article in "best of the Forum" re 5spds (and diesels). I have a Clark 280VO overdrive 5spd behind my stock bellhousing in the stock locat1on (attached to Hercules diesel, however) in my M37. It has 5th = 0.799:1. A NP540 will also fit, 5th = 0.821:1. Some machine work on the bellhousing will probably be required. The little driveshaft can be shortened and used for either of these two gearboxes. A few NP540 cases may have been made which bolt up to flatheads, they may have been for early W500s before they changed to OHV motors.
Don't count on finding one of these cases, feel fortunate if you are able to find any overdrive 5 spd in good condition for a good price.
A NV4500 5 spd will probably be too long unless you are willing to switch to a married NP205 TC which then may require either a centered rear diff or semi exotic modifications to change it to offset rear output, using some parts of your old NP200 to hybridize the two cases.
