Great write up Keith.
I have had the Death Wobble three times in my 2nd gen Dodge CTD, and it is extremely scary.
I have read about this problem extensively on the TDR forum and magazine.
With the 2nd gen Dodges, it is frequently due from the weak track bar design they used then.
But many components can cause it in any type truck, it just means than an oscillation is set in motion. Any loose steering/front axle components can lead to it, especially in a 4X4 with big tires and being naturally high up.
The manufacturers euphemistically call it a "shimmy". I do not know anyone who has experienced it who calls it that.
In my case a new 3rd generation track bar retrofit kit was the primary cure. These are available from various vendors. Mine was made by Solid Steel if I recall.
