I don't think it much matters. It's easier to turn the engine by hand if you have all the plugs out. The main thing is to make sure all the valves are closed on the cylinder you're testing. Also, if it's not right at TDC, the air pressure will tend to turn the engine over. If the engine tries to rotate, try turning it back a little by hand to get it right at TDC. They sell leakdown testers, which are basically a gauge to give a relative indication of how fast a cylinder is leaking and let you compare them, but serious problems will show up pretty easily even without a gauge. Basically you'll be listening for leaks and then if you hear any air hissing, try to figure out where it's going. You can put your hand over the exhaust pipe, crankcase breather/filler, or intake manifold to verify it that's where the air is getting out. Air leaking into the water jacket will make bubbles in the radiator or blow water out of it. Expect a small amount of leakage, especially through the valves. The main thing is if one cylinder is clearly worse than the others.
