I do not think that there are 5 full twists of the axle. I do not think that is possible. However, the break is right at the end of the splines. Right were the stresses are concentrated. Were the splines cut into the shaft? If so that would possibly explain the break. There is another way to make splines besides cutting them and results in much less stress concentration that cutting them causes.
