Hi David,
I have taken them to large tire (truck and ag tires) places and had them do it. and have done it myself at my shop. I used the bucket of my backhoe to push down on the outer ring, that allows the inner ring to come free. You could probably rig up something on a hydraulic press too.
Once you get the rings off, and push the rim out of the tire you will find the run flat inside. It is a large donut of solid rubber. Guys have offered other methods for getting them out, but the only way I have had luck with is to sawsall it through, being careful not to cut the inside of the tire, and corkscrew it out. There is enough room to get in there with the sawsall safely, IF you are careful. The run flat and inside of the tire are coated with a slimy afterbirth like goo. not real pleasant but it cleans up fine.
