Yep bent disk, disk install backwards or incorrect throwout bearing or not on fork correctly. Also check to see if the center hub of the disk is tight and the rivets are tight. Most disks are rebuilt and the hub can/will be loose. That is not good. Mine came apart there and I could not shift. The first one I got from an auto parts store was loose. The spare I had taken out years ago and kept and was loose. I ended up not having a core.

I replaced my clutch disk this last summer in my ffPW. I had the same setup as you. It was very hard to find the correct disk. I actually ended up at a large clutch shop to get it.
