Sounds like the steering knuckle bushings are shot, which would explain both symptoms. As the bushings wear the axle tube drops in relation to the steering knuckle, and stresses the tracta joint too (not good).

Jack up the axle and grab the tire at 6 and 12 o'clock. If the tire and knuckle wobble a little you may be able to adjust out the play. A lot of movement means it's time for a new set. You can either install new bronze bushings (PW and M37 bushings interchange) or use timken roller bearing for a very smooth steering experience.

If you find you have no play in the knuckle, it may be that your spring shackle pins and/or bushings are worn.
