Most common prob is the pin in the tower has worn. In my truck the pin, which should be about 11/32" diameter, had been replaced with a grade 8, 5/16 bolt that had it's head welded in. Bolt body broke off, and shifting was like stirring a pot of soup. All tightened up by grinding off the bolt head, removing the old stub of bolt, and driving in a tight pin.
I think more often, the slot in the ball is worn more than the pin. May need a bit of welding and a file to restore the slot.
