Your correct the front axle engages as you 'pull" the high/low lever back.
Just because the hubs are not engaged the T-case can still go in low range.
Make sure both levers are forward for high two wheel drive.
Then check to see if the cab has settled on it's mount's more since you drove it last.
If the cab is touching the frame or cross member anywhere it will greatly amplify the noise.
If this checks out then follow Dave's advice on rebuilding.
