I am with Jeff suspecting a fuel issue.

1. Your gas cap must be VENTED. Otherwise you will draw a vacuum on the tank and not get fuel flow. The distance you are driving before failure is about right for a vacuum.
If you have replaced your gas cap, it may not be the correct one with a vent. If you have not replaced the gas cap, the vent may be plugged. Either try another gas cap that you KNOW is vented or put the gas cap on, but leave it loose so air can vent in.

If this does not solve the problem

2. Rig a small tank up front with the line going directly to the fuel pump. If it runs well no, you know the problem is either the fuel line to the tank or the pickup in the tank is full of crud.

Both are easy and free things to try. I bet one of them solves your problem.
