I have a 4bt in my m37 ( I guess it is an 8 valve) which is very similiar to the first gen cummins. The only somewhat major thing to look for with the engine is the thrust bearing especially with a manual tranny. They come apart and then you wind up with a ridiculous amount of crank endplay, the good news is the bearing can be replaced without too much trouble and in my case it has worked fine. I wish I could tell you how to find this but my engine had no outward sign until I pulled the oil pan to clearance it to fit in my truck. I have heard that some have issues with the clutch disengagement. Maybe someone else here has experienced this and could help with what to look for without pulling the oil pan. The other thing on early diesel pickups is the trannsmissions were not really strong enough I think the manual options are better but even the manuals have some known weakness.
