they dont come loose. inner nut adjusts bearings and has pin for lock ring, outer nut is tightened to what ever the book says. works great. more durable than the sheetmetal tab type ones. The washer is nice and thick so wont shear the little tooth off and come looselike the sheet metal ones do. I believe larger trucks still use this design.
