I find I have an increasingly short fuse for dealing with underlings who can't solve the problem. As soon as it's clear that they don't have either the authority or the will to do the right thing, I start going right up the ladder until I get someone who can do it.

My letters to corporate always point out that the trouble I'm having must be due to one of two things. Either the underling correctly understood corporate policy and was accurately informing me of it, or they were not properly trained. I also point out that at this point, solving the problem is going to cost a lot more in the time and energy of higher-paid people than it would have cost for a properly-trained flunky to handle it correctly in the first place. So far, they've always come through once I got up to somebody who was above the level of "the computer won't let me" or "I have to follow procedures, SIR."
