Call larry Cook at CC Ring and Pinion, Corpus Christi Texas, he rebuilds manual 540s still, that adapter should still be avialable new from the pto maker or from NP, you need to go to a HD truck outfitter shop, I have the 4 speed in my w500 with a pto but have not looked to see if it uses a adapter, do you still have the picture of the offset adapter you need, i am going by his shop this wedesday????
