Did you check T137 for rear PTO part numbers?

As mentioned in the posts below, there is still a current number for the bearing that you need. This is only the beginning though. You mention that you have the special offset bracket #1189 445. Do you have the cap for the bracket #562 117? You will also need:

Center Bearing MUD SLINGER
Center Bearing HOUSING
Center Bearing Housing INSULATOR
Center Bearing OIL SEAL
Center Bearing Retainer SPACER
Center Bearing Retainer NUT
Center Bearing Retainer Nut LOCK

You can see a description of all parts with part numbers and a cross section view at Phil's T137 link below. Some of these parts are not unique and can be found on two-piece drive shafts from other larger Dodge trucks if you can not find them new.
