For some reason, it sounds like your not getting enough throw on the clutch throwout bearing/release arm. Is your clutch set up as fully mechanical or hydraulic. Are all parts stock,ie; meaning absolutely correct parts for your PW ? Wrong Clutch Disk or Clutch Pressure Plate or throwout bearing could cause this problem.

No, you can not leave washers between the clutch pressure plate and flywheel. The clutch pressure plate must sit square and even on the flywheel, with all contact points
on the on the flywheel. There is tremendous force when the clutch pedal is pushed to move the clutch pressure plate. Not sitting square with all points in contact could/would cause distoration and even more problems. Not to mention a possible ballance issue.

Need to know more about your set up including answers to the above questions.
