I think the honest answer is sometimes you can't get the door to fit perfectly no matter what you do. I've had to try and bend the whole door to try and make it fit the cab. I've widened slots in the door hinges and bent them around with hammers. Like Paul said it can take hours to get one to fit properly, it can be very frustrating. The drivers door on my truck still doesn't fit like I want it to. Sounds like you have one of those problem doors, good luck with it.
