... consulting with an Osteopathic Doctor (DO) and ask about OMT (Osteopathic Manipulative Treatment, I think). Not the same thing as you, but my wife had horrible back pain when she was pregnant, and the DO here basically cured her when the other docs said that nothing could be done.
Also, if you are near a teaching hospital, you might ask if they have a Chronic Pain Management study going on, and if you can be a subject. There are supposedly a number of new techniques being developed right now. Your regular doctor might be able to help you with this.
